Sakichi Toyoda, fondatore della Toyota Motor Company, ha sviluppato il processo "5 Whys", un tipo di analisi di causa radicale, per identificare e risolvere i problemi che sono sorti all'interno dell'azienda. Da allora è stata ampiamente adottata come parte della metodologia produttiva magra, Kaizen e Six Sigma.
